Please follow these important instructions to expedite your booking:

1. 1. Attach your requisition below:

   Make sure the image is clear and easy to read. Blurry or distorted images cannot be processed.
2. 2. Multiple scans:

   We’ll book your first scan now. Any additional scans can be scheduled after your first visit.
3. 3. Contact information:

   Please include your current cell phone number on the requisition so we can send you text updates about your booking.
4. 4. Confirmation:

   Once we receive your requisition, you’ll get an email or text message from our office to complete your booking.
5. 5. Avoid delays:

   Please do not resend the same requisition. Submitting multiple requests for the same exam will delay processing.

Use your confirmation email for any future communication with us.
Include your name and PHN for authentication.
